Tutorial Allocation Change Request Form 2009

Complete this form only if you are unable to make the requested change yourself on the ALLOC8 system and you have a valid reason for requesting the change. A valid reason is either (1). Childcare (2). Medical and (3). Religious. We will do our best to make the necessary change, however even with a valid reason a change may not be possible. Work, travel or inconvenience are not valid reasons and changes requested for these reasons will not be processed.

Note: Not all lectures and tutorials listed on the SIS website are running, therefore please ensure that you request to change to only those appearing on the ALLOC8 system.
